Work Environment\n

This position is primarily remote, allowing for flexibility while providing support to our hospice center. The ability to work from home or another remote setting is designed to facilitate work-life balance. However, you may be required to visit the hospice center for client assessments or participate in team meetings. Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to understand and evaluate patients’ emotional, social, and financial needs.

• Work together with interdisciplinary medical teams to devise and implement care plans that address the holistic needs of hospice patients.

• Provide individual and group counseling sessions, focusing on adjustment to illness, grief, and coping strategies.
\n
• Coordinate post-discharge services with community organizations, ensuring seamless care continuity after patients leave the hospice setting.
\n
• Offer emotional support and education to patients and their families regarding treatment options, resources, and ways to cope during difficult times.
\n
• Participate in hospital committees, contributing your expertise in social work to enhance patient care processes.
\n
• Develop detailed discharge plans that connect patients with relevant community resources to support their ongoing needs after leaving hospice care.

• Document all client interactions and treatment plans accurately and in a timely manner, in compliance with legal and hospital requirements.

Patient Population / Client Focus\n

Your primary focus will be on patients in hospice care, many of whom are dealing with terminal illnesses. This role demands not only clinical expertise but also a deep sense of empathy and understanding of the emotional intricacies involved in end-of-life care. You will work closely with patients facing significant life changes, as well as their families, providing support during a profoundly challenging time.

Qualifications\n

• A Master's degree in Social Work from an accredited program.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
