Unit Information:

• Units are mainly 20 beds each.
• Team of CM/SW to manage load
• Type of staff: Day Shift Friday-Monday 8-4:30 Day Shift Monday Friday 8-4:30 Night Shift N/A
• Patient Ratios SW: 1:40 RN: 1:20
• Type of equipment: mobile, computer EMR Meditech / CarePort

Typical Hiring Profile:

• Skill Set: Most recruited for: (Travel RNs, local CNAs and sitters)
• Must have: candidates without these skills will not be considered for the role. Minimum 1-year current acute care Case Management RN experience
• Preferred or nice to have: candidates with these skills will be considered first. Discharge Planning ED experience Palliative Care Knowledge and GOC Conversation
• List typical procedures performed on unit(s): Discharge Planning Assessments
• Best personality fit: Professional and dedicated and friendly Strong communication skills Adaptable to different units.
