LPN Profession: Licensed Practical Nurse Specialty: Long-Term Care

The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must demonstrate knowledge of the principles of growth and development across the lifespan. The LPN should possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to age-specific needs. The LPN will provide direct and indirect skilled nursing care for assigned residents. The LPN will follow defined policies and procedures under the supervision of a Registered Nurse. The LPN must maintain a collaborative relationship with other nursing staff, physicians, and ancillary staff. Maintaining an appropriate level of knowledge and clinical expertise through continuing education and in-service programs is essential. The LPN shall have the ability to provide care to geriatric residents. The LPN will collect data relevant to the resident's physical and psychosocial condition on an ongoing basis and communicate findings to the RN and other members of the Interdisciplinary Team. The LPN will contribute to the creation of a developmentally age-appropriate and culturally relevant plan of resident care. This plan will reflect nursing diagnoses and interventions to achieve desired resident outcomes. The LPN will involve the resident/family and healthcare team in this process while promoting a healthy and safe environment. Revisions to the plan of care may be recommended after evaluating resident outcomes. The LPN will indicate the need for referrals to ensure all aspects of the resident's care have been addressed. Collaboration with the resident/family and members of the Interdisciplinary Team is vital within the scope of nursing practice to assure customer satisfaction. The LPN must maintain current knowledge and competency in Nursing Practice. Participation in the mentoring/orientation of staff is encouraged. The LPN will consider factors related to safety, cost, and impact when planning the delivery of care. Under the direction of the RN, the LPN will provide leadership in the professional practice setting. Demonstrating knowledge and competency is required. Occupational exposure to bloodborne pathogens is reasonably anticipated. Required tasks will involve potential contact with blood, body fluids, or tissues.
