Part-Time Home Health Physical Therapist

Join our caring community as a Home Health Physical Therapist, reporting to the Clinical Manager, to help patients regain strength, mobility, and independence in their homes. Plan, administer, and document personalized physical therapy treatments to improve patients' physical function. Assess patients' strengths, develop rehabilitation programs aligned with physician plans, and support team goals. Provide clear instructions and oversee performance of PTAs and Home Health Aides, ensuring quality care. Maintain thorough documentation of interventions, evaluations, and patient progress. Collaborate with healthcare professionals and communicate patient updates to the care team, patients, and families. Qualifications Include Active and unrestricted Physical Therapy license. CPR certification. Experience working with healthcare teams. Preferred: At least one year of experience as a Physical Therapist.
