Registered Nurse - Home Health & Hospice

Position Overview:
Seeking a compassionate and experienced Hospice Registered Nurse to provide high-quality, patient-centered care to individuals receiving end-of-life services. This role involves coordinating care, supporting patients and families, and working collaboratively with an interdisciplinary team across Northwest Arkansas service areas.

Schedule & Work Details:

• Location: Fort Smith, Arkansas service area
• Coverage Area: Northwest Arkansas including Sebastian, Logan, Crawford, Franklin, Scott, and Yell counties
• Schedule: Monday through Friday, 8:00 AM to 5:00 PM
• Weekly Hour Guarantee: 32 hours
• On-Call/Weekend Requirement: Participation in back-up on-call rotation
• Mileage Reimbursement: Yes, reimbursed between patient visits (first patient to last patient)
• Pay Rate: $51 to $53 per hour

Key Responsibilities:

• Provide skilled nursing care to hospice patients in home and facility settings
• Perform comprehensive patient assessments and develop individualized care plans
• Administer medications, treatments, and interventions in accordance with physician orders
• Monitor patient condition and adjust care plans as needed
• Educate patients and families on disease progression, symptom management, and end-of-life care
• Collaborate with physicians, social workers, chaplains, and other interdisciplinary team members
• Maintain accurate and timely clinical documentation
• Participate in on-call rotation to support patient needs after hours
• Ensure compliance with all regulatory and organizational standards

Required Qualifications:

• Active Registered Nurse (RN) license in the state of Arkansas or compact license
• Minimum of 1 to 2 years of clinical nursing experience
• Prior experience in hospice, home health, or palliative care preferred
• Strong clinical assessment and critical thinking skills
• Valid driver's license and reliable transportation
• Ability to travel throughout assigned coverage areas
• Current BLS certification

Preferred Qualifications:

• Previous hospice experience strongly preferred
• Experience with electronic medical records (EMR) systems
• Excellent communication and interpersonal skills
• Ability to work independently and manage time effectively
• Compassionate, patient-focused approach to care
• Strong organizational and documentation skills
