Job Description:

Responsibilities:

• Provide direct patient care focused on comfort and symptom management.
• Develop and implement care plans with the hospice team.
• Educate patients and families on disease processes and end-of-life care.
• Coordinate care with interdisciplinary team members and community resources.
• Administer medications and monitor for side effects.
• Maintain accurate patient records.
• Offer emotional support to patients and families.
• Be available for on-call care when needed.

Qualifications:

• CT Registered Nurse license
• BLS/CPR Certification
• Minimum 1-2 years' experience in hospice, palliative care, or nursing in a similar healthcare setting is required
• Valid driver's license

Job details:

• Pay: $1,799.01 - $1,854.65 / week
• Location: Greenwich, CT
• Assignment Length: 13
• Shift: Day - up to 36.00 hours per week
• Setting: Skilled Nursing Facility - SNF
• Workplace: On-site
