Position Overview

This position requires a valid California CNA License and involves working collaboratively with nursing staff, residents, families, and various facility departments to provide optimal individualized care for residents in a Skilled Nursing Facility (SNF). The role involves shifts that may include days, nights, weekends, and holidays, with a potential requirement to float to other units as necessary.

Key Responsibilities

• Receive and give report on residents' conditions, ensuring clarity on the care to be provided.
• Conduct rounds on each resident, assess all new residents, and monitor those who are ill, have fallen, or have sustained injuries.
• Document assessments per facility policy.
• Notify nursing staff of any changes in residents' conditions or needs.
• Assist with direct patient care, ensuring that call lights are answered promptly.
• Foster good working relationships with other departments, coworkers, residents, families, and physicians.
• Perform other duties as assigned.

Qualifications

• Must possess a valid California CNA License.
• Excellent communication skills and ability to work effectively within a team.
• Compassionate and attentive to the needs of residents.
