CNA - LTC/SNF

Job Summary:
Provides direct patient care to residents in a skilled nursing facility, ensuring their individual needs are met with compassion and professionalism. Collaborates with nursing staff, residents, families, and other departments to deliver high-quality, personalized care.

Responsibilities:

• Receive and relay clear reports on residents' conditions to nursing staff.
• Conduct rounds on residents, assessing new admissions, residents who are ill, have fallen, or sustained injuries.
• Document assessments accurately following facility policies.
• Notify nurses promptly of any changes in residents’ conditions or needs.
• Assist with direct patient care, ensuring call lights are answered quickly and residents are comfortable.
• Foster positive relationships with other departments, staff, residents, families, and physicians.

Qualifications:

• Current California CNA license required.
• Proven experience working in a skilled nursing or long-term care setting.
• Strong communication skills and ability to collaborate effectively.
• Ability to work flexible shifts, including weekends and holidays.
• Demonstrated competence in resident assessments and documentation.
• Team-oriented with a compassionate approach to resident care.
