Physical Therapist – Home Health – (PT – HH) Start Date: ASAP. Available Shifts: 8 D. Pay Rate: $2,384.00 – $2,456.00. This Home Health Physical Therapist opportunity is based in Portland, Oregon. The role serves patients throughout the broader Portland metro area, providing one‑on‑one care in patients’ residences, focusing on restoring mobility, improving safety, and promoting independence. You will perform comprehensive evaluations, develop individualized treatment plans, and deliver evidence‑based interventions tailored to each patient’s functional goals and home environment. Typical caseloads include adult and geriatric patients recovering from surgery, managing chronic conditions, or transitioning from inpatient facilities to home. Your day will involve traveling between patient homes, coordinating with a supportive interdisciplinary team, and documenting visits using an electronic medical record system. Shifts are primarily daytime hours with flexibility. Weekend or on‑call rotations may be required depending on agency practices but are usually scheduled in advance to support work‑life balance.
