This is a permanent position with salary and benefits.About the Opportunity Our client, a leading healthcare provider in Southern California, is seeking a dedicated Nurse Practitioner to provide high-quality care to high-risk patients in the San Fernando Valley region. This position offers the opportunity to make a meaningful impact by managing post-discharge care for patients with complex conditions, including Congestive Heart Failure (CHF) and Chronic Obstructive Pulmonary Disease (COPD), through home and clinic-based services.Key ResponsibilitiesProvide comprehensive care to high-risk patients in home and clinic settings.Follow evidence-based clinical guidelines to ensure quality patient care.Document all patient interactions and care provided in accordance with regulatory standards.Coordinate follow-up care, including access to social and behavioral health services.Monitor and document clinical and social issues for patients.Conduct all visits in a timely manner, adhering to group and health plan protocols.Perform additional duties as assigned.QualificationsCurrent certification as a Nurse Practitioner.Minimum of 3-4 years in healthcare, with specific experience managing COPD and CHF patients.Experience with high-risk, complex patient populations preferred.Home care experience is a plus.Excellent verbal and written communication skills in English (bilingual in Spanish preferred but not required).Proficiency in Microsoft Office applications.Ability to work independently while meeting team deadlines.Compensation and BenefitsSalary: $140,000 - $160,000 annually, based on qualifications, market location, experience, and skills.Benefits Include: Employer-paid medical, pharmacy, dental, and vision insurance with zero co-pay for employed physician office visits.Flexible Spending Account (FSA) and employer-paid life insurance.Employee Assistance Program (EAP) and 401k retirement savings plan.Generous paid time off including vacation, sick leave, and holidays.Tuition reimbursement, license renewal support, and company celebrations.
