Job Description
Registered Nurse (RN) - Care Manager

We are seeking a dedicated and experienced Registered Nurse (RN) to join our Care Management team. This position plays a crucial role in ensuring effective discharge planning and care coordination for our patients.

Position Details

Shift: Day Shift (7:30 AM - 4:00 PM)
Weekend Requirement: Every third weekend required, with occasional weekend hours available but not mandatory
Holiday Requirement: Working a holiday required

Key Responsibilities

Manage discharge planning and care coordination effectively.
Collaborate with multidisciplinary teams to provide comprehensive patient care.
Adapt to various needs within the Care Management department.

Qualifications

Experience: Minimum of 5 years in nursing with a focus on care management preferred.
Education: Nursing degree from an accredited program.
Skills: Demonstrated abilities in discharge planning and care coordination.
Personality Fit: Must be flexible, easy-going, a problem solver, and possess a calm demeanor with a positive attitude.

Additional Information

Patient Ratios: 1:20-24
Float Requirement: Occasional floating within the Care Management department as needed.
Dress Code: Any scrubs or professional casual dress are acceptable.
