Registered Nurse - Hospice Case Manager (Paso Robles and SLO Areas)

Aevum Hospice is seeking a compassionate and experienced Registered Nurse (RN) Case Manager to join our hospice care team. The ideal candidate has hospice experience, strong case-management skills, and a genuine commitment to supporting patients and families during end-of-life care.
Key Responsibilities
Manage and coordinate care for an assigned hospice patient caseload
Complete comprehensive nursing assessments and develop individualized plans of care
Monitor and document changes in condition, symptom burden, functional status, and disease progression
Provide pain and symptom-management interventions in collaboration with the hospice physician and interdisciplinary team
Educate and support patients, families, caregivers, and facility staff
Coordinate medications, medical equipment, supplies, and other hospice services
Communicate effectively with physicians, facilities, pharmacies, and members of the interdisciplinary team
Participate in interdisciplinary group meetings and assist with care planning
Complete accurate, timely, and Medicare-compliant clinical documentation
Participate in the on-call rotation as assigned
Qualifications
Current and unrestricted California Registered Nurse license
Previous hospice nursing experience strongly preferred
Case-management experience required
Knowledge of hospice eligibility, plans of care, recertification, and Medicare documentation requirements
Strong clinical assessment, communication, organization, and time-management skills
Ability to work independently while remaining engaged with the interdisciplinary team
Valid driver’s license, reliable transportation, and current automobile insurance
Current CPR certification
Compassionate, dependable, and comfortable caring for patients with advanced and terminal illnesses
