Social Worker - Case Management

Medical Social Worker
Schedule: Shift: 5x8 Days
Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: 72 hours per 2-week period
Requirements:
- Minimum experience: 1 year in acute care
- Required clinical experience: discharge planning / care coordination
- Preferred experience: diverse case management across Med-Surg, LLOS, ED
- Licensure: Must hold a clinical license in the state
- Certifications: BLS
- Charting: EPIC experience required
Roles & Responsibilities:
- Conduct biopsychosocial assessments for patients.
- Develop and implement appropriate plans of care.
- Coordinate discharge planning and referrals to home care, DME, rehab, and long-term care.
- Assist patients and families with understanding Medicare/Medicaid processes.
- Provide support for high-risk and vulnerable patients.
Compliance / Notes:
- Orientation: 40 hours required
- Candidates may be required to work 3 of the 5 major holidays.
- Background checks and references required prior to start.
