Patient Care CoordinatorFull-time: 40 hours per week. M-F 8a-4pRemoteJob Summary: We are seeking a detail-oriented and organized Patient Coordinator to coordinate and manage the scheduling of infusion therapy services for our patients. The ideal candidate will possess excellent communication skills, a strong understanding of infusion therapy, and the ability to work collaboratively with healthcare teams to ensure timely and effective patient care.Key Responsibilities:Office Administration: Answer phonesReceive and manage faxes and emailsHelp facilitate communication of supply lists between RNs and pharmaciesScheduling Coordination: Manage and coordinate the scheduling of infusion therapy appointments for patients.Collaborate with nursing staff and other healthcare providers to optimize appointment availability.Help facilitate deliveries by the pharmacies to the patientsPatient Communication: Communicate with patients to confirm appointments, confirm receipt of supplies and drug, and address any non-clinical questions or concerns.Ensure patients are aware of their treatment schedules and any necessary follow-up.Record Keeping: Maintain accurate and up-to-date patient records in the electronic health record (EHR) system.Document scheduling changes and communicate updates to relevant team members.Collaboration: Work closely with infusion nurses and medical staff to ensure appropriate staffing levels for scheduled procedures.Participate in team meetings to discuss scheduling challenges and develop solutions.Qualifications:Education:High school diploma or equivalent requiredCollege degree preferredExperience:Healthcare coordination experience requiredPrior experience in infusion therapy or clinical scheduling strongly preferred.Familiarity with an electronic health record (EHR) system, preferably AlayacareSkills:Strong organizational and time-management skills.Excellent communication and interpersonal skills.Ability to work independently and as part of a team.Working Conditions:This is a remote position
