Mobile Nurse Practitioner – LA County & Orange County

A nationally recognized healthcare organization is seeking a Mobile Nurse Practitioner to provide acute, post-acute, and transitional care to medically complex and recently discharged patients throughout the LA County & Orange County, California .

This is an excellent opportunity for a Nurse Practitioner who enjoys autonomous clinical practice, home-based care, and building meaningful relationships with patients . You'll work directly in post acute facilities while collaborating with an interdisciplinary care team focused on improving outcomes, preventing unnecessary hospitalizations, and helping patients safely recover at home.

As a Mobile Nurse Practitioner, you'll have the opportunity to make a meaningful impact by meeting patients where they are most comfortable—their homes.

You'll care for patients who are transitioning home following hospitalization or skilled nursing facility stays, as well as individuals managing complex chronic conditions. This field-based position provides the opportunity to truly understand each patient's environment, support system, and unique healthcare needs.

You'll have significant clinical autonomy while remaining closely connected to physicians, nurses, care managers, pharmacists, social workers, behavioral health specialists, and other members of the care team.

How You'll Make an Impact

As a Mobile Nurse Practitioner, you will:

• Provide acute, post-acute, and transitional care directly in patients' homes
• Evaluate medically complex and recently discharged patients
• Perform comprehensive physical assessments
• Diagnose and treat acute and chronic medical conditions
• Develop individualized treatment and care plans
• Manage medications and coordinate appropriate follow-up care
• Identify potential complications and intervene early
• Help reduce unnecessary hospitalizations and readmissions
• Educate patients and families regarding disease management and preventive care
• Coordinate care with physicians, nurses, pharmacists, social workers, and other providers
• Communicate with hospitals and skilled nursing facilities during patient transitions
• Maintain accurate and timely clinical documentation
• Utilize clinical judgment and work independently in a field-based environment
• Participate in an interdisciplinary approach to patient care

What We're Looking For

Minimum Qualifications

• Master of Science in Nursing ( MSN )
• Adult/Geriatric Nurse Practitioner certification
• Active, unrestricted Registered Nurse (RN) license
• Active, unrestricted Nurse Practitioner license in California
• Minimum of 1 year of Nurse Practitioner clinical experience
• Strong assessment, diagnostic, and care management skills
• Excellent communication and interpersonal skills
• Ability to work independently while collaborating with a multidisciplinary team
• Valid driver's license
• Reliable transportation for home visits
• Current TB test or willingness to obtain prior to employment
