Physical Therapist Assistant (Home Health) Part Time, Days

We are seeking a dedicated and patient‑focused Part‑Time Physical Therapist Assistant to join our supportive and collaborative care team. This unique role offers flexible scheduling options that support work‑life balance while ensuring continuity of patient care. Responsibilities Carry out individualized treatment plans under the direction of a licensed Physical Therapist. Provide hands‑on therapy interventions and contribute to patient progress through skilled observation and communication. Perform reassessments and adjust treatment plans according to state and facility guidelines. Select appropriate treatment methods based on patient diagnosis, age group, physical and emotional status. Notify primary PT and case manager of non‑routine changes in treatment situation, discharge complications, or other circumstances requiring reassessment or staff guidance. Instruct ancillary staff in patient positioning, mobility and caregiving. Educate patients and/or families on home exercise programs, transfers, caregiving, home safety and use of adaptive equipment. Adhere to department policy for documentation, including daily notes that reflect patient response, performance, and assessment of status. Document reasons for canceled treatments and provide clear, concise, legible records within established time frames. Communicate effectively with patients, families, medical staff, care team members, students, supervisors and outside contacts to facilitate treatment plans. Provide educational materials and involve family/caregiver in training as appropriate. Give appropriate instruction and supervision to assistive personnel. Complete all administrative responsibilities, maintain productivity standards, and minimize non‑billable time. Contribute to educational and training efforts of the department/clinic (e.g., in‑services, student, new hire orientation). Demonstrate fiscal responsibility and knowledge of department expenses and revenue. Qualifications Associate’s Degree from a Physical Therapist Assistant program. Michigan Physical Therapist Assistant License. BLS for Healthcare Provider by 60 days after hire. One year of applicable experience preferred. Basic computer knowledge. Completion of initial and ongoing stroke care education required. Demonstrated essential skill in the use of treatment modalities, equipment and protocols. Schedule & Compensation Employment Type: Part Time, 40 hours every 2 weeks. Schedule: Days, No weekends, Rotating Holidays.
