Housing Case Manager - Outreach & Permanent Housing
Kids for the Future
City Net is seeking a Case Manager to provide housing-focused case management and supportive services to individuals experiencing homelessness, primarily through street outreach. You will meet clients where they are to help develop long-term housing plans and connect them with resources for medical care, counseling, and permanent housing. The role follows a four/10 schedule, working Wednesday through Saturday, 7am–5:30pm, and requires strong data collection, advocacy, and collaboration with #J-18808-Ljbffr Kids for the Future
