HonorHealth is excited to open a new on-site clinic at Sagewood Senior Living, providing high-quality, patient-centered care exclusively for the residents of this vibrant senior living community.

We are seeking a compassionate and experienced Part-Time Nurse Practitioner to join our team. This is an excellent opportunity to help establish a new clinic while building meaningful relationships with older adults in a collaborative, supportive environment. As the clinic grows, there is the potential for this position to transition into a full-time opportunity .

Position Highlights

• Provide comprehensive primary care for Sagewood residents.

• Deliver same-day sick visits and urgent care for acute medical needs.

• Develop lasting relationships with patients through continuity of care.

• Collaborate with HonorHealth physicians, nursing staff, and the Sagewood care team.

Qualifications

\n
• Current Nurse Practitioner license in Arizona.
\n
• National board certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP).
\n
• Required: Primary care experience.
\n
• Required: Experience caring for an older adult/geriatric patient population.
\n
• Excellent clinical judgment, communication, and interpersonal skills.
\n
• Passion for providing compassionate, patient-centered care to seniors.
\n
