Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of seniors, evaluating emotional, mental, and social factors impacting their well-being.

• Collaborate with seniors and their families to create personalized care plans that meet individual needs and goals.

• Provide counseling and emotional support, assisting clients in navigating transitions and challenges such as health issues or changes in living conditions.

• Advocate for clients’ rights, ensuring they have access to necessary community resources and services.

• Engage with interdisciplinary teams, coordinating efforts among healthcare professionals to enhance clients' overall care experience.

• Conduct regular virtual or in-home visits to monitor clients' well-being, assess living situations, and address emerging concerns.

• Maintain detailed documentation of client interactions, assessments, care plans, and progress to ensure compliance and quality of care.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Active and valid state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years of experience in a social work capacity, preferably within a community center or geriatric care environment.

Compensation and Benefits\n

The salary range for this position is competitive, offering between USD 80,000 and 104,000 annually, commensurate with experience.
