Physical Therapist

Job Summary:

We are looking for a compassionate and skilled Physical Therapist (PT) to join our interdisciplinary team in a Skilled Nursing Facility (SNF). The Physical Therapist will evaluate residents' physical conditions, develop personalized treatment plans, and deliver rehabilitative care to enhance mobility, restore function, and promote independence.

Key Responsibilities:

• Conduct thorough assessments of patients' physical abilities, movement limitations, and pain levels.
• Develop and implement individualized treatment plans based on evaluation findings, medical history, and patient goals.
• Deliver evidence-based physical therapy interventions to enhance strength, coordination, balance, and mobility.
• Collaborate with physicians, nursing staff, occupational and speech therapists, and family members to coordinate patient care.
• Monitor patient progress, adjust therapy plans as required, and document all care provided in accordance with regulatory guidelines.
• Educate patients and caregivers on safe mobility techniques, home exercises, and equipment usage.
• Ensure compliance with all regulatory requirements, including documentation standards and facility policies.
• Participate in care planning meetings and interdisciplinary team conferences.
• Supervise physical therapist assistants (PTAs) and aides as needed.
• Maintain a safe and clean work environment.

Qualifications:

• Graduate of an accredited Physical Therapy program.
• Current state license to practice as a Physical Therapist (PT).
• CPR certification (preferred or required depending on the facility).
• Experience in a Skilled Nursing Facility or geriatric rehabilitation setting preferred.
• Excellent communication, documentation, and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.

Work Environment:

The position is based in a Skilled Nursing Facility (SNF) that provides inpatient, long-term care, and short-term rehabilitation services. The role may involve physical activities such as lifting, standing, walking, and assisting patients with transfers and mobility.

Shift Information:

Standard working hours are from 9 AM to 5 PM.

Minimum Years of Experience:

Less than 1 year is required for this position.
