Position Summary This position requires the full understanding and active participation in fulfilling the mission of Orange Coast Memorial Medical Center. The employee shall support Orange Coast Memorial Medical Center’s strategic plan and participate in and advocate performance improvement/patient safety activities. The position requires seven days a week including holidays (major and minor holidays) “on call” coverage on a rotating basis as determined by the Social Work Services Manager and the needs of the department. Essential Functions and Responsibilities Conduct psycho‑social assessments and interventions with patients and families. Address psycho‑social issues and concerns through the continuum of care. Navigate systems and advocate to assist patient in accessing appropriate resources and services. Make mandated protective services reports based on psycho‑social assessment of patient and family. Address barriers to a safe and timely discharge. Be present and punctual; required to be on campus during scheduled core business hours to accommodate crisis issues and support the medical team, peers, and staff in a specific area. Follow company policies, procedures, and directives. Interact in a positive and constructive manner. Prioritize and multitask effectively. Perform other duties as assigned. Qualifications Qualifications/Work Experience: 1 year or less of direct social work experience in an acute healthcare setting preferred. Competency is defined and measured via the Social Worker Job Specific Competency Assessment and Checklist. Each Social Worker, MSW (New Grad) is assessed at orientation and annually using these tools. Education / Licensure / Certification: Master of Social Work (MSW) Degree from an accredited School of Social Work required.
