Description

• Conduct thorough evaluations for patients in both inpatient and outpatient settings and formulate tailored treatment plans
• Deliver exemplary physical therapy interventions that target mobility, strength, balance, and overall function
• Engage in collaboration with nurses, physicians, case managers, and a diverse range of rehabilitation professionals to enhance patient outcomes
• Educate both patients and their families regarding therapeutic goals, discharge processes, and appropriate home exercises
• Maintain accurate documentation of evaluations, treatments, and patient progress in line with facility policies and regulatory standards
• Take part in multidisciplinary team rounds and interdisciplinary meetings to ensure seamless care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification highly preferred
• Any specialized certifications (such as geriatric or neurological) are advantageous but not mandatory

Skills

• Preferred prior experience within a clinical setting
