Description

• Conduct comprehensive evaluations for patients in both inpatient and outpatient settings while creating tailored treatment plans.
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function.
• Collaborate with nurses, physicians, case managers, and other rehabilitation specialists to achieve the best possible patient outcomes.
• Instruct patients and their families on therapeutic goals, discharge plans, and exercises for home care.
• Ensure accurate documentation of evaluations, treatments, and patient progress according to regulatory standards.
• Engage actively in care team discussions and interdisciplinary conferences to promote coordinated patient care.

Education

• Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution.

Certifications

• Current state licensure as a Physical Therapist.
• CPR certification is preferred.
• Additional certifications in areas such as neurology or geriatrics are advantageous but not mandatory.

Skills

• Previous experience is strongly desired.
• Capable of managing patients with complex medical and mobility challenges.
