This position ensures compliance with federal, state, and local regulations while maintaining adherence to the National Association of Social Workers (NASW) Code of Ethics. Key Responsibilities Resident Assessment & Care Planning Complete comprehensive social histories and psychosocial assessments for residents. Identify emotional, psychological, social, and environmental needs. Complete and maintain relevant Minimum Data Set (MDS) documentation. Utilize assessment findings to develop individualized care plans. Participate in interdisciplinary care plan meetings and resident reviews. Resident & Family Support Provide counseling, support, and therapeutic interventions to assist residents with adjustment and transition to long-term care. Educate residents, families, and significant others regarding placement, care planning, and available resources. Assist residents in exercising their rights and supporting self-determination. Coordinate referrals to community resources and outside service providers when needed. Discharge Planning & Advocacy Coordinate discharge planning and transitions of care. Arrange appropriate referrals for home health and community services. Promote and protect resident rights. Assist in identifying and addressing concerns related to abuse, neglect, and psychosocial well-being in accordance with regulatory requirements. Compliance & Documentation Maintain accurate, timely, and compliant documentation including: Social Service Assessments Care Plans Progress Notes Behavior Monitoring PASARR Documentation Advance Directives Discharge Planning Records Psychosocial and Cognitive Status Tracking Ensure compliance with all federal, state, and facility regulations. Team Collaboration Work closely with nursing, therapy, administration, physicians, residents, and families. Assist with survey preparation and regulatory compliance activities. Participate in quality improvement initiatives and interdisciplinary team meetings. Support the development, implementation, and evaluation of the facility's social services program. Qualifications Required Licensed, certified, or registered in the State of Missouri as required by professional practice standards. Bachelor's Degree in Social Work or Human Services field, including: Social Work Sociology Psychology Rehabilitation Counseling Special Education Related Human Services discipline Minimum one (1) year of supervised social work experience in a healthcare setting serving geriatric populations. Strong knowledge of resident rights, care planning, discharge planning, and long-term care regulations. Preferred Experience in a Skilled Nursing Facility (SNF) or Long-Term Care setting. Minimum two (2) years of experience working within post-acute or long-term care environments. Knowledge of MDS, Medicare, Medicaid, and state survey processes.
