Position Overview

Bridge Rehabilitation Center is seeking a compassionate and dependable Registered Nurse (RN) licensed in Michigan to perform nursing assessments and skilled nursing visits for clients receiving rehabilitation and care coordination services.

We are particularly seeking an RN who can conduct post-operative nursing assessments and evaluate clients before rehabilitation services begin. This role is ideal for a nurse looking for flexible scheduling and competitive per-visit compensation.

Responsibilities

Conduct initial nursing assessments for new clients.
Perform post-surgical and post-hospital discharge assessments.
Assess overall health status, medications, vital signs, and recovery progress.
Identify nursing needs, safety concerns, and barriers to recovery.
Provide education to clients and caregivers regarding post-operative care.
Document findings and recommendations accurately and timely.
Communicate assessment findings with the interdisciplinary care team.
Monitor changes in condition and recommend appropriate interventions or follow-up care.

Qualifications

Active Michigan Registered Nurse (RN) License .
Current BLS/CPR certification preferred.
Minimum 1 year of clinical nursing experience preferred.
Home Health, Rehabilitation, Case Management, Med-Surg, or Post-Acute Care experience preferred.
Strong assessment, communication, and documentation skills.
Reliable transportation for client visits.
