Social Worker Team Lead

Givens Estates is looking for a full-time Social Work Team Lead to support our Resident Services team. Reporting to our Resident Services Director, this role will support the social work team across all levels of care on campus.

What you'll do:

• Support residents and families through transitions between care levels, crisis situations, and changes in health or cognition
• Facilitate advance care planning, mental health referrals, and family meetings as needed
• Provide day-to-day mentorship to social work staff serving each level of care
• Conduct regular team meetings and interdisciplinary case reviews
• Partner with Life Enrichment, Nursing, Wellness, Therapy, and Chaplaincy to deliver integrated services that support psychosocial well-being
• Assign and monitor caseloads to ensure equitable distribution and continuity of care
• Perform any other duties and/or tasks that may be assigned on an as-needed basis by an authorized supervisor

What you'll need:

• Master's degree in social work (MSW) from an accredited program strongly preferred
• Licensed Clinical Social Worker (LCSW) or equivalent state licensure strongly preferred
• Minimum 5 years of social work experience with older adults; at least 2 years in a supervisory or lead role preferred
• Experience in senior living, long-term care, hospice, or hospital discharge planning preferred

Compensation is $73,000 - $78,000 per year, based on background and experience, and includes our comprehensive benefits package
