Care Coordinator (Wraparound)

Care Coordinator (Wraparound) Job Code: 2025-SPECIALI-020 | Location: Milwaukee | Department: Wraparound | Status: Full Time Position Summary Working under the Wraparound Practice Model of care, which emphasizes individualized, strength-based, trauma‑informed, community‑based and culturally intelligent care, you will provide intensive and individualized care coordination for children and adolescents. Essential Functions Maintains a caseload of approximately 8 children and families. Contact enrolled children and families within 48 hours of enrollment and schedule an initial visit within 5 business days. Schedule and attend weekly visits and facilitate monthly Child and Family Team Meetings for treatment planning, and maintain regular phone contact with families and service providers as needed. Assemble a Child and Family Team within two weeks of enrollment by interviewing the family and identifying family members, natural supports, service providers, and other significant persons. Within the first 30 days, coordinate the meeting of the Child and Family Team, develop the initial individualized Plan of Care based on identified strengths and needs, and include a comprehensive 24‑hour Crisis Plan. Monitor service provision and quality, acting as liaison for new service/resource development. Advocate for children and families across home, educational, court, and community settings as needed. Attend all scheduled court hearings as a representative of Wraparound Milwaukee to support children and families throughout the court process. Seek community resources with the team and modify the SAR whenever services or resources need to be added or deleted. Provide or arrange transportation for youth/young adults to appointments, crisis/respite services, including assisting families with natural support and transportation plans. Provide or secure support and crisis/emergency services for youths and/or their families via face‑to‑face, phone, or on‑call systems. Complete all required paperwork in a strength‑based manner (court letters, Plans of Care, placement forms, SARs, referrals, progress notes, evaluation instruments, consent forms, etc.). Maintain accurate online information, ensuring family demographics and team member contact details are up‑to‑date. Collaborate with other individuals who may interact with the youth and family. Report emergencies or critical incidents to Wraparound Milwaukee in accordance with Policy #14. Other Duties and Responsibilities Attend in‑services and participate in staffing, weekly/monthly meetings, and consultations as needed. Assist with child and family coverage for co‑workers as directed. Perform other job‑related duties necessary to carry out the responsibilities of the position. Job Qualifications Minimal Qualifications Bachelor’s degree in a relevant area of education or human services and at least one year of continued experience providing mental health services. Completion of 85 hours of mandatory Wraparound philosophy and policy training and attendance at a Family Orientation within six months of hire. Working knowledge of computer applications (Windows, Microsoft Word) preferred. Demonstrated cultural competence and respect for diversity across cultures, languages, races, ethnic backgrounds, religions, and other factors. Knowledge and skills to work with children, young adults, and families. Valid driver’s license, automobile, and insurance sufficient to meet agency requirements. Meet all employee requirements, including references, criminal background check, drug test, and driver’s license check. Knowledge, Skills, and Abilities Deeper understanding of positive child/youth development; patience with traumatic life situations; ability to interact with escalating youth and/or parents calmly; ability to follow oral and written instructions; crisis response skills; proficiency with Microsoft Word and Excel; basic computer functions; efficient time management; multi‑tasking; sensitivity to cultural, ethnic and disability issues. Work Relationship and Scope Reports directly to the Wraparound Supervisor. Has contact with a wide variety of individuals including clients, family members, program staff, consulting psychologists/psychiatrists, collateral contacts, community agencies, and the public. Working Conditions Work is performed primarily in a busy office managing caseloads of children, young adults and families, as well as in the community and families’ homes. Work includes sitting at a desk, driving (several hours per day), and flexible hours that may extend into evenings and weekends.
