Responsibilities

• Provide direct nursing care to residents under supervision of an RN, including routine assessments and postoperative checks as assigned.
• Administer medications and treatments per physician orders and facility policy, documenting administration accurately.
• Monitor residents’ health status, respond to changes, and escalate concerns to the supervising RN promptly.
• Support activities of daily living, including personal care, mobility assistance, and patient safety measures.
• Maintain clear, compliant documentation in the electronic health record and ensure consistency of care across shifts.
• Collaborate with the care team to plan and implement individualized care plans, noting progress and any barriers to care.

Requirements

• Current LPN license for Pennsylvania (active or valid multi-state license as applicable)
• Ability to work in a long-term care setting and adapt to a dynamic resident population
• Strong communication and teamwork skills with the interdisciplinary care team
• Reliable, punctual, and able to work evenings on a 5x8 schedule
• Familiarity with basic nursing procedures, documentation, and resident safety protocols
