Nutrition Coordinator

Summary The Nutrition Coordinator will be working directly with UCHC’s Registered Dietitian staff. The coordinator is responsible for client enrollments, taking patient vitals, following up referrals, orientations, scheduling patients, patient questionnaires, and appointment reminders. This position involves identifying nutrition and other health‑related referrals, performing client risk assessments, and providing basic nutrition education in individual and group settings. Description Job Title: Nutrition Coordinator Department: Nutrition Reports to: Registered Dietitian FLSA Status: Non‑Exempt General Accountabilities Welcome patients who contact the clinic personally or by telephone; answer or refer inquiries and explain available services, payment options, and billing procedures. Schedule appointments; handle warm hand‑offs and referrals based on established policies and procedures. Answer all incoming calls and assist patients with their requests to transfer to the appropriate department. Register all patients per registration protocol, collect all documentation, verify, update, and input information into EMR while maintaining confidentiality of personal and financial information. Work closely with physicians, physician assistants, medical assistants, and staff to assure smooth patient flow and reduce waiting time. Assist patients in distress by responding to emergencies. Call and remind patients of their appointments; follow up on “no‑show” patients daily. Conduct nutrition assessments and provide nutrition counseling to clients. Screen for health and nutritional risk factors and special nutritional needs of patients. Assist in the development of individual nutrition plans based on client abilities, lifestyle, budget, and culture; refer clients to other services. Advocate breastfeeding and provide breastfeeding assessment, education, and referrals. Determine and certify client eligibility for the WIC federal supplemental food program. Provide nutrition and breastfeeding education to WIC clients; plan and develop nutrition education experiences and activities. Produce nutrition educational materials, handouts, and newsletters for clients. Communicate patient problems or complaints to the office manager or designee. Maintain operations by following policies and procedures and reporting needed changes. Manage fax work, including making copies, sending, and receiving faxes. Ensure the working area is clean and orderly. Perform diagnostic procedures to screen for nutritional health problems; take weight and height measures and enter data into state databases. Interview patients and conduct interactions. Identify nutritional needs associated with low income, cultural influences, life stages, and medical status; use effective client‑centered counseling and interview techniques; screen clients for income and residency eligibility; interpret policies. Other related duties as the job requires. The company reserves the right to add or change duties at any time. Requirements Job Qualifications Education: High School Diploma or equivalent required. Excellent phone communication and customer service skills; ability to work well with constant interruptions and relate to patients; familiarity with medical terminology and triage procedure. eClinicalWorks experience: 1 year (Preferred). Patient interaction experience: 1 year (Preferred). Data analytics or referrals in healthcare experience: 1 year (Preferred). Required Certifications: BLS certification (AHA) required. Skills Ability to work under pressure and treat all patients with utmost kindness and consideration. Friendly personality and desire to work with the public. Ability to handle multiple functions. Understanding of community‑based organizations. Initiative and problem‑solving skills. Excellent verbal and written communication skills. Excellent phone skills. Excellent organizational and prioritizing skills. Professional presentation of self and company. Proficiency with personal computers, corresponding programs, and internet literacy. Attention to detail. Interpersonal and customer service skills. Extensive knowledge of Adobe Acrobat, MS Word, and Excel. Language Skills Bilingual in English/Spanish preferred. Physical Requirement Physical effort may include occasional light lifting to a 25‑pound limit, bending, stooping, or squatting. Considerable walking may be involved. Ability to sit or stand for extended periods of time is required. Flexibility to work in multiple locations throughout the week. Physical demands described are representative of those that must be met to perform essential functions. Reasonable accommodations can be made for disabilities. May be required to travel to all UCHC facilities as needed. Occasionally required to sit and walk. Finger dexterity and hand coordination required. Specific vision abilities required: close vision, distance vision, ability to adjust or focus. Work Environment Exposure to adverse conditions. Exposure to extreme heat, cold, wet/humid conditions, moving mechanical parts, high precarious places. Exposure to fumes, airborne particles, toxic or caustic chemicals, outside weather conditions. Possible risk of electrical shock, explosives, radiation, vibration. Protective clothing or equipment required: gloves, helmets, steel‑toed boots, protective eyewear. Work environment noise level usually is high.
