Clinical Quality Coordinator II - Quality Administration

POSITION SUMMARY

To be fully engaged in providing Quality/No Harm, Customer Experience, and Stewardship by measuring the functioning of important processes and services, identifying changes that enhance performance, ensuring the improvements are sustained, and maintaining a continuous level of patient safety and accreditation readiness with regulatory agencies.

PRIMARY ACCOUNTABILITIES

Works closely and effectively with all members of the Quality and Outcomes Management team, and collaboratively with associates of all disciplines to ensure patient safety and quality.

QUALITY/NO HARM

Coordinates and provides direction to the facility's quality committees, including medical staff, nursing and hospital-based committees or others as assigned, by providing accurate and timely information and written communication. Acts as chair or facilitator. Documents and disseminates minutes. Prepares agendas. Follows up on open items. Accurately reports to other committees as required. Provides accurate and timely clinical data to appropriate committees or groups and identifies opportunities for improvements. Collects data or oversees processes. Aggregates and analyzes data. Provides benchmarks. Verifies statistical measures. Oversees collection, analysis and preparation of data for entry into specific databases and maintains the integrity of the information. Acts as a resource for data abstraction and analysis as requested by departments and the medical staff. Performs analysis, reviews and summaries, and facilitates proper use of information. Maintains all hard copy and/or electronic files for a period of seven years related to accreditation standards and Performance Improvement activities. Maintains responsibility for knowledge and skill of clinical applications. Coordinates and provides support to the clinical peer review process by providing accurate and timely information for quality initiatives. Handles accurate and timely medical records requests. Handles accurate and timely chart review and documentation. Follows up with specific physicians. Develops focus studies as needed. Documents to the Quality Improvement Organizations (QIOs) if required. Facilitates Quality Improvement Teams per policy. Is knowledgeable of team models. Provides education to team members and staff. Acts as team facilitator or leader as requested. Provides oversight for the Department Indicator monitoring and performance measurement for corporate and facilities goals. Acts as a resource with regulatory agencies, makes recommendations for compliance, and prepares mandatory requirements as required. Ensures Joint Commission Readiness. Complies with Florida Medical Quality Assurance, Inc. (FMQAI). Complies with Centers for Medicare and Medicaid Services (CMS). Complies with Agency for Health Care Administration (AHCA). Proactively seeks opportunities to identify and reduce risks to the safety of patients. Is knowledgeable of Failure Modes and Effects Analysis (FMEA) and Root Cause Analysis (RCA) processes. Utilizes documentation templates for the FMEA and RCA. Participates in risk reduction strategies.

CUSTOMER EXPERIENCE

Respects and assists all associates and customers appropriately. Maintains patient confidentiality as per professional ethics and HIPAA mandates.

STEWARDSHIP

Works within budgetary constraints. WORK EXPERIENCE & QUQualifications REQUIRED RN with at least 5 years acute care clinical experience. Minimum of 4 years experience in clinical settings in quality improvement. Current license to practice nursing in the State of Florida required. Bachelor's degree in nursing or related science preferred. Demonstrated proficiency with computer software, word processing, graphics, spreadsheet, GroupWise and Microsoft Office. Presentation skills and knowledge of presentation software required. Strong interpersonal, communication, organizational skills. Ability to maintain positive working relationships with all levels within the organization. Analytical abilities required to recognize and resolve problems requiring technical or clinical interventions. Able to prioritize work and manage time. Able to work flexible hours for early and late meetings.

PHYSICAL DEMANDS

Ability to sit for long periods of time and perform typing skills using hand dexterity up to two hours at a time, 6-8 hours per day. Ability to work flexible and/or long hours as required. Must be able to bend and reach at all times. Must be able to carry a minimum of 20 lbs. Must have visual acuity/near depth perception, visual acuity/far depth perception at all times. Must be able to hear and speak clearly.

MENTAL DEMANDS

Ability to coordinate multiple tasks on an ongoing basis. Ability to adapt to changing organizational requirements and business applications. Ability to identify priorities.
