Key Job Details – Pediatric RN Case Manager Employment Type: Travel Contract Healthcare Setting: Pediatric Home Care (Certified Home Health Agency - CHHA) Location: Webster, NY 14580 (Rochester area, New York healthcare jobs) Estimated weekly salary: $2,250 - $2,310 Schedule: 3 shifts per week, 8 hours per shift (Friday – Sunday, 8:00am – 4:30pm). Training for the first two weeks: Monday – Friday, 8:00am – 4:30pm. Contract Duration: Approximately 65 weeks (Start: 09/15/2025 – End: 12/13/2026) RN Case Manager Qualifications Active New York State Registered Nurse (RN) license Minimum 1 year of clinical nursing experience; pediatric nursing and/or home care experience preferred Certified Home Health Care experience required Proficiency with OASIS assessments and telemetry Experience with EPIC or Netsmart EMR preferred BLS (any provider accepted) and ACLS (AHA only) certifications required Valid NYS driver’s license and current auto insurance Ability to travel throughout Webster and the greater Rochester, NY area Strong interviewing, assessment, and communication skills Proficiency with Microsoft Office Suite and electronic medical records RN Case Manager Responsibilities Develop and implement individualized pediatric care plans with patients, families, physicians, and the clinical team Complete OASIS assessments to accurately reflect clinical, functional, and social needs Ensure timely and accurate completion of all clinical records for compliance with state and federal regulations Coordinate and communicate with healthcare team members and community resources for seamless patient care Maintain professionalism, courtesy, and flexibility in all patient interactions Meet productivity standards and adhere to home visit performance guidelines Exercise independent judgment and critical thinking to deliver safe, effective pediatric home care Travel to patient homes throughout Webster and the Rochester, NY region Discover the charm of Webster, NY—where "Life is Worth Living"—and enjoy easy access to Lake Ontario, scenic trails, and a thriving community while advancing your nursing career in pediatric home health care.
