This position offers the opportunity to develop long-term relationships with patients while delivering high-quality, patient-centered care in a rewarding assisted living setting. Position Options Part-Time 2–3 full days per week Travel between Hoosick Falls, NY and Cambridge, NY Full-Time 4–5 full days per week Travel between Hoosick Falls, NY, Cambridge, NY, and Troy, NY Requirements Qualifications Required Active New York State Physician License (MD or DO) Active DEA Registration Currently enrolled and credentialed with Medicare and Medicaid Primary Care experience Preferred Geriatric Medicine experience Assisted Living, Long-Term Care, or Skilled Nursing Facility experience Benefits Compensation & Benefits Competitive compensation package $5,000 retention bonus after 90 days Flexible scheduling options Supportive clinical environment Anticipated start date within 4–6 weeks Important Requirement Candidates must already be actively enrolled and credentialed with Medicare and Medicaid.
