Subacute Hospitalist Director

Job ID: 1138797 Opportunity: Subacute Hospitalist Director Specialty: Family Medicine, Hospitalist, Internal Medicine Profession: Physician (MD, DO) Responsibilities Oversee the delivery of high-quality medical care across affiliated Skilled Nursing Facilities. Manage post‑acute and chronic medical conditions and develop and supervise patient care plans. Collaborate with interdisciplinary teams to ensure exceptional patient outcomes and continuity of care. Provide leadership and oversight for subacute providers, promote clinical quality initiatives, and support operational excellence. Qualifications Board eligible or board certified in Internal Medicine or Family Medicine. Licensure or eligibility for licensure in the State of New Jersey. Additional training and/or certification in Geriatrics preferred. Flexibility to travel between Skilled Nursing Facilities and participate in a shared group call model. Minimum 5+ years of clinical experience. Strong clinical leadership experience driving improvements in quality and safety. Commitment to academic excellence, including prior experience in resident education and clinical research. ABIM certified. Exceptional interpersonal skills and strong clinical background, with a keen understanding of driving and maintaining clinical quality standards. Compensation The anticipated salary range for this position is $175,000 – $385,000 per year, depending on experience and qualifications.
