For the past four years, we’ve partnered with top‑tier nursing and skilled care facilities nationwide to manage pain and enhance therapy outcomes, helping thousands of patients regain independence and comfort. We’re Hiring: We are seeking a full‑time Nurse Practitioner or Physician Assistant experienced in Skilled Nursing Facilities to join our team as a Regional Lead. This role combines clinical practice, regional oversight, and business development, responsible for ensuring clinical excellence and growth. The ideal candidate will be a strong clinician, mentor, and relationship builder in skilled nursing. Day‑to‑Day Responsibilities Must live in one of the following locations: Albuquerque, NM; Tacoma, WA; Seattle, WA; Oakland, CA; or Sacramento, CA. Primarily responsible for regional oversight of clinical care and facility relationships; also rounds 2–3 days a week at your own designated facility to care for patients experiencing difficulty participating in therapy due to pain or functional limitations. Clinical point of contact for all providers in your region. Clinical training to ensure providers display excellent clinical decision‑making and bedside manner. Time shadowing each provider in the region. Regular clinical and facility meetings with facility staff. Focused assessments and development of individualized treatment plans in collaboration with facility primary care teams to optimize therapy participation and functional recovery. Providing targeted interventions such as joint injections, Botox treatments, dry needling, and medication management based on patient‑specific needs. Supporting physiatrists on‑site and operating with a high level of autonomy. Coordinating care and referring to other specialties or services for issues beyond the scope of physiatry. This role is best suited for a self‑motivated and confident clinician who thrives in independent practice environments and is passionate about improving rehabilitation outcomes. Qualifications 5+ years as an NP or PA. Current, unrestricted license as an NP or PA. DEA licensed or willing to obtain DEA licensure. Skilled Nursing Facility experience. Physiatry, Pain Management or Physical Medicine & Rehabilitation experience. Comfort performing ultrasound‑guided peripheral joint injections (knees, shoulders, hips). Genuine passion for providing professional, personalized care in an autonomous environment. Relationship and value‑proposition building with key stakeholders. Strong communication skills, with the ability to collaborate with multidisciplinary teams. Experience in a clinical leadership or supervisory role. Willingness to travel regularly within the assigned region. Benefits Hands‑on training in facility settings to ensure consistent, high‑quality care. Stable schedule, with no weekends, nights or on‑call shifts. Gain valuable experience in joint injections and rehabilitation care. Competitive salary and bonus structure (starting base of $150,000+/year depending on experience). Comprehensive benefits package: medical, dental, vision, life insurance (we cover a $50,000 policy with the option to add additional coverage), and AD&D insurance. 8 paid PTO days, 7 paid sick leave days, and 6 paid federal holidays – 21 total paid days off. Paid malpractice insurance ($1M/$3M policy). License renewal allowance (up to $1,100/year). Full autonomy in patient care with direct impact on patient outcomes at multiple facilities. Immediate Hiring Timeline We’re looking to make hiring decisions soon, with a start date in 4–6 weeks following insurance and Medicare credentialing. Job Type: Full‑time Pay: $145,000–175,000 base salary plus bonus, with total compensation potentially $185,000–205,000 USD plus performance‑based incentives.
