Action Wellness started as ActionAIDS in 1986 when a group of dedicated volunteers came together to support their community at the beginning of the HIV epidemic. Today, staff provide a range of trauma-informed services to help persons with HIV and individuals with substance use disorders at risk of acquiring HIV achieve physical, emotional and economic well-being.

Job Summary

The Prison Linkage Program Case Manager is responsible for delivering comprehensive case management services to individuals currently incarcerated in, or recently released from, the Philadelphia County Prison system. This position plays a critical role in supporting clients through the transition from incarceration to community life, with a focus on reducing reoffending, improving health outcomes, and fostering long-term stability.

Essential Functions

• Conduct in-person needs assessments to identify client strengths, challenges, and service requirements.
• Manage incoming referrals from various sources to connect incarcerated individuals with appropriate services.
• Collect and update medical and demographic information, securing informed consent from each client as required.
• Prepare and update service care plans 1x per six (6) months.
• Gather and document updated client medical and demographic details, ensuring informed consent is obtained.
• Maintain accurate, up-to-date activity logs in accordance with program requirements.
• Deliver case management and advocacy services to both HIV-positive individuals who are incarcerated or recently released, including in-person visits at prisons, homes, hospitals, and the office.
• Assist clients in accessing medical care, social services, and other provider systems by facilitating negotiations, arranging linkages, and providing information and referrals. Coordinate necessary services, conduct monthly follow-ups, and reassess client needs as appropriate.
• Partner with healthcare teams to ensure smooth hospital discharges and arrange appropriate home care services.
• Deliver specialized case management and advocacy for clients primarily experiencing symptoms of HIV, while supporting successful reentry into society following incarceration.
• Evaluate and coordinate client entry into appropriate substance use or mental health treatment services.
• Identify and liaise with service providers supporting individuals during and after incarceration.
• Coordinate and oversee service delivery while supporting clients in advocating for benefits and entitlements.
• Work in partnership with other service providers and community agencies.
• Collaborate with Action Wellness team members, including direct service staff, housing specialists, volunteers, and other relevant personnel as needed.
• Manage a caseload of 40–45 HIV-positive clients, providing the full spectrum of case management support.
• Complete intake documentation and information/referral inquiries within 24 hours.
• Complete required paperwork for client interactions and referrals in accordance with established Direct Service policy deadlines.
• Maintain documentation of client interactions in accordance with the minimum contact standards and timelines outlined in the Direct Service Policy and Procedure Manual.
• Ensure all performance measures—such as client contact, screenings, and service plan completion—are consistently met.
• Attend at least one medical visit per year with each client to ensure continuity of care.
• May supervise up to one Care Outreach Specialist.
• Attend Direct Service and General Staff meetings.
• Participate in monthly team group supervision meetings.
• Attend required meetings, including Direct Services, in-service trainings, and general staff gatherings.
• Attend weekly supervisory meetings to review progress, receive guidance, and ensure accountability.

Education and Experience

• Bachelor’s degree in Social Work, Human Services, Nursing, or a related field required. 
• Minimum of two (2) to three (3) years’ experience in case management, social services, or healthcare, preferably working with individuals living with HIV/AIDS and mental health conditions.
• Experience working with justice‑involved individuals, including those currently incarcerated or recently released.
• Experience in care coordination, client advocacy, and resource navigation within diverse and underserved communities.
• Proficiency in addressing mental health, developmental challenges, substance use issues, and providing effective case management and advocacy.
• Familiarity with correctional systems, probation/parole processes, and community‑based service providers.

Work Location, Hybrid Information & Pay Range 

Based out of our Center City office, Hybrid remote after 6 months 

Annual pay range: $44,000-49,000
