Responsibilities
- Perform comprehensive occupational therapy evaluations for residents in the SNF setting
- Develop and implement individualized, occupation-based treatment plans to improve activities of daily living, mobility, and cognitive function
- Deliver direct therapy and adapt interventions to maximize resident independence and safety
- Collaborate with the interdisciplinary team to plan discharge and transition of care
- document progress accurately in the electronic medical record and communicate changes to the care team
- Ensure compliance with regulatory standards and facility policies while maintaining patient and family education

Requirements
- Active Occupational Therapist license in New Hampshire (or eligibility for licensure in NH)
- Experience or demonstrated capability in acute care or skilled nursing settings (preferred)
- Ability to work in a skilled nursing facility environment and complete documentation in the electronic medical record
- Strong communication skills and ability to collaborate with an interdisciplinary care team
