Position Summary

The Rural Health Transformation – Mobile & Grassroots Project Manager is responsible for leading JHN’s rural health transformation initiatives, with a primary focus on bringing healthcare directly into underserved and rural South Carolina communities.

This position manages the day-to-day implementation of JHN’s mobile health, pop-up clinic, community outreach, and grassroots health initiatives. The Project Manager will coordinate staff, community partners, mobile operations, outreach schedules, grant deliverables, data collection, budgets, and program outcomes.

The ideal candidate is highly organized but also comfortable working directly in the community. This is not solely a desk-based project management position. The Project Manager is expected to help build relationships, identify communities in need, organize services, and ensure JHN delivers on its rural health commitments.

Key Responsibilities

Project & Grant Management

• Lead implementation of JHN’s Rural Health Transformation initiatives.
• Develop project timelines, implementation plans, milestones, and measurable deliverables.
• Ensure grant-funded activities are completed on schedule and within approved scope.
• Coordinate project activities across clinical, outreach, administrative, finance, pharmacy, and community partners.
• Maintain organized documentation supporting grant activities and deliverables.
• Prepare regular project updates for JHN leadership.
• Assist with grant progress reports, performance reports, and required documentation.
• Identify project barriers and develop solutions to keep implementation on schedule.

Mobile Health Operations

• Coordinate JHN mobile health activities throughout targeted rural communities.
• Develop and maintain the mobile health outreach calendar.
• Identify strategic locations for mobile clinics, screenings, pop-up clinics, and community health events.
• Coordinate staffing, equipment, supplies, transportation, technology, and community logistics.
• Ensure mobile and field operations are prepared before deployment.
• Work with clinical leadership to ensure appropriate services, protocols, and staffing are available.
• Monitor mobile health utilization and identify opportunities to increase community participation.

Grassroots Community Outreach

• Build JHN’s grassroots presence in rural communities.
• Develop relationships with trusted community leaders and organizations.
• Conduct community outreach to identify healthcare gaps and emerging needs.
• Coordinate health screenings, educational events, community conversations, and resource distribution.
• Help connect individuals with healthcare regardless of insurance status or traditional barriers to care.
• Promote JHN services through direct community engagement.
• Ensure JHN maintains a visible, trusted, and consistent presence within participating communities.

Community Partnerships

Develop and maintain partnerships with organizations including:

• Pharmacies
• Free clinics
• Rural healthcare providers
• Hospitals and health systems
• Churches and faith communities
• Recovery organizations
• Food distribution programs
• Community centers
• Schools and colleges
• Local governments
• Housing organizations
• Social service organizations
• Community health workers
• State and local public health agencies

The Project Manager will help ensure partnerships result in measurable services and outcomes rather than simply organizational relationships.

Rural Health Services Coordination

Work with JHN clinical and outreach teams to coordinate access to services such as:

• HIV screening
• Hepatitis C screening
• STI screening and treatment
• PrEP and HIV prevention
• Doxy-PEP education and access
• Diabetes/A1C screening
• Blood pressure screening
• Chronic disease screening
• Telehealth
• Primary care navigation
• Medication access
• Pharmacy services
• Insurance and benefits navigation
• Community Health Worker services
• Transportation and social-support referrals
• Linkage and retention in healthcare

Patient Navigation & Linkage

• Develop systems to ensure individuals identified through outreach do not become lost to follow-up.
• Coordinate referrals between outreach teams, clinical staff, Community Health Workers, pharmacies, and community partners.
• Track referrals from initial screening through successful linkage.
• Help remove transportation, insurance, medication, scheduling, and other barriers to care.
• Monitor individuals requiring follow-up and ensure appropriate team members are assigned.

Data & Performance Management

Maintain accurate project data and monitor key performance indicators, including:

• Number of communities served
• Number of mobile/pop-up events conducted
• Number of individuals reached
• Number of individuals screened
• HIV, Hepatitis C, STI, and chronic disease screenings
• New diagnoses/reactive screenings
• Linkage-to-care rates
• PrEP referrals and starts
• Hepatitis C treatment referrals and starts
• Primary care referrals
• Telehealth encounters
• Community partnerships established
• Rural counties reached
• Patient follow-up and retention

The Project Manager will work with JHN leadership to turn this information into meaningful reports demonstrating community impact.

Budget & Fiscal Responsibilities

• Monitor project spending against approved grant budgets.
• Work with JHN finance staff to ensure expenses are properly allocated to the appropriate grant or program.
• Review project purchases and supply needs.
• Maintain documentation for grant-funded purchases and activities.
• Assist leadership with budget-to-actual reviews.
• Help forecast future project expenses.
• Follow JHN procurement and financial policies.
• Ensure grant resources are used appropriately and efficiently.

Compliance & Documentation

• Maintain organized project records for monitoring and audit purposes.
• Follow applicable grant requirements and JHN policies.
• Ensure patient information is handled in accordance with HIPAA requirements.
• Work with leadership to ensure grant-funded activities follow applicable federal, state, and funding-source requirements.
• Maintain supporting documentation for program activities, expenses, partnerships, and outcomes.
• Assist with preparation for grant monitoring, audits, and program reviews.

Staff & Team Coordination

• Coordinate rural outreach teams, Community Health Workers, clinical staff, contractors, volunteers, and community partners.
• Conduct project planning meetings.
• Clearly communicate assignments, deadlines, locations, and deliverables.
• Monitor completion of assigned project activities.
• Help identify staffing and training needs.
• Create a culture of accountability, teamwork, flexibility, and community service.

Physical & Travel Requirements

This position requires regular travel throughout South Carolina and participation in community events, mobile health deployments, and outreach activities.

The employee must be able to perform reasonable field-based activities associated with setting up community events, transporting outreach materials, and supporting mobile health operations, with reasonable accommodations provided as required by law.

Mission

The Rural Health Transformation – Mobile & Grassroots Project Manager will help JHN build a sustainable model that takes healthcare beyond the traditional clinic walls and directly into communities where access is limited.

The goal is not simply to conduct events—it is to create lasting pathways connecting rural South Carolinians to screening, treatment, medication, primary care, prevention, and ongoing healthcare.

Requirements: Qualifications

Required:

• Experience in project management, healthcare, public health, nonprofit programs, community outreach, social services, or a related area.
• Strong organizational and project-management skills.
• Strong written and verbal communication skills.
• Ability to manage multiple projects and deadlines simultaneously.
• Comfortable working directly with rural and underserved communities.
• Ability to develop strong community partnerships.
• Ability to track data and prepare program reports.
• Valid driver's license and reliable transportation.
• Ability to travel throughout South Carolina.
• Ability to work occasional evenings and weekends based on outreach schedules.

Preferred:

• Bachelor's degree in Public Health, Healthcare Administration, Business Administration, Social Work, Project Management, or related field.
• Experience managing grant-funded programs.
• Experience with rural healthcare.
• Community Health Worker experience.
• Mobile health or community-clinic experience.
• Experience with HIV/STI/Hepatitis C programs.
• Experience managing budgets and grant deliverables.
• Familiarity with federally funded programs and 2 CFR Part 200 requirements.
• Experience working with healthcare data, electronic health records, or program-tracking systems.
