Director of Rehab (DOR) – Lead Our In-House Therapy Team!  

Location: Saint Albans, VT 

Position: Full-Time (FT) 

Setting: In-House Skilled Nursing Facility (SNF)

We are looking for a passionate Director of Rehab to lead our clinical team, foster a positive culture, and drive outstanding patient outcomes. What You’ll Be Doing:  

As our DOR, you will be the captain of our therapy department, balancing clinical excellence with operational success. 

• Lead & Inspire: Manage, mentor, and support a multidisciplinary team of PTs, OTs, SLPs, and assistants, fostering a collaborative and highly engaged work environment. 

• Program Development: Drive clinical programming and community integration initiatives to ensure our patients achieve their highest level of independence. 

• Operations & Compliance: Oversee daily department operations, manage caseloads, and ensure strict compliance with Medicare guidelines, state/federal regulations, and facility protocols. 

• Interdisciplinary Collaboration: Work hand-in-hand with facility leadership, nursing staff, and admissions to ensure seamless patient transitions and top-tier care. 

• Tech & Reporting: Utilize EMR and management systems (experience with platforms like Net Health or PointClickCare is a huge plus!) to track clinical success and department efficiency. 

What You Bring to the Table:  

• Education & Licensure: Active state license as a Physical Therapist (PT), Occupational Therapist (OT), Speech-Language Pathologist (SLP), Physical Therapist Assistant (PTA), or Certified Occupational Therapy Assistant (COTA) in VT. 

• Experience: Proven clinical experience in a SNF or adult rehab setting. Previous management or DOR experience is highly preferred. 

• Knowledge: Strong understanding of Medicare regulations, PDPM, and ethical reimbursement practices. 

• Skills: Exceptional leadership capabilities, strategic problem-solving skills, and a genuine passion for staff development and geriatric care.
