MDS Nurse Assessment Coordinator for AFC in Brockton ($100)

Registered Nurse (RN)/Case ManagerBrockton Home Health AFC is looking for a Registered Nurse (RN)/Case Manager to manage and support our Adult Family/Foster to make sure the programs run smoothly.About the Position: We are seeking a caring and detail-oriented Registered Nurse to join our Adult Foster Care (AFC) program as an Initial Assessment / Admission Nurse. In this role, you will complete in-home evaluations for individuals applying to the AFC program, reviewing their needs and confirming eligibility.
