This full-time position, based remotely, is rooted in our outpatient practice located in Elmira, New York. As a vital member of our multidisciplinary team, you will leverage your expertise to develop holistic treatment plans and provide individualized care to our patients.

Work Environment\nThis role will primarily be conducted in a remote setting with the support of advanced electronic health record (EHR) systems, ensuring that you remain connected with colleagues and patients alike. Our standard working hours are Monday to Friday from 8:00 AM to 5:00 PM, with occasional evening shifts to accommodate patient needs.
Key Responsibilities\n- \n
- Conduct comprehensive mental health assessments that take into account the intricate interactions between medical conditions and mental wellness. \n
- Design and implement integrated treatment plans that respond to both psychiatric and medical needs, utilizing evidence-based therapeutic interventions. \n
- Facilitate group and individual therapy sessions tailored to the unique requirements of each patient, ensuring a supportive environment conducive to healing. \n
- Collaborate closely with physicians, nurses, and other medical professionals to coordinate care and enhance treatment outcomes across disciplines. \n
- Provide crisis intervention strategies and immediate support to patients during mental health crises, ensuring their safety and well-being. \n
- Educate and empower patients by conveying vital information regarding the connections between mental and physical health, aiding them in managing their conditions more effectively. \n
- Maintain precise and confidential patient documentation, adhering to legal and ethical standards to uphold patient rights and trust. \n
- Engage in team meetings and case discussions to continuously improve treatment approaches and outcomes, fostering a spirit of collaboration among team members. \n
- Stay updated on the latest research and best practices related to integrated care to improve treatment strategies and patient care methodologies within the organization. \n
In this role, you will primarily serve a diverse population of patients facing mental health challenges alongside chronic medical issues. This includes individuals experiencing high levels of stress due to their health conditions, as well as those who may struggle with substance use disorders. Your contributions will directly enhance their treatment experiences, promoting holistic health care that addresses the full spectrum of their needs.
Qualifications\n- \n
- A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, Psychology, or a closely related field from an accredited institution. \n
- A current and valid license as an LCSW, LCPC, LCP, LSCSW, or LMFT in New York State. \n
- Demonstrated proficiency in addressing the mental health needs of individuals with co-occurring medical conditions. \n
- Experience in delivering integrated care and collaborating with multidisciplinary medical teams. \n
- Exceptional interpersonal and communication skills, capable of establishing rapport with patients from various backgrounds. \n
- A strong adherence to ethical guidelines, maintaining professional boundaries and patient confidentiality at all times. \n
- Familiarity with electronic health record systems and other relevant technologies for patient management. \n
- Proven ability to function effectively within a collaborative interdisciplinary team. \n

- At least two years of experience in a similar role, ideally within an integrated care or psychiatric outpatient setting. \n
- Previous involvement in team-based care initiatives or programs focused on improving patient outcomes in complex cases. \n
- A background in crisis intervention and emergency mental health support. \n
