The Community Health Nurse is responsible for providing comprehensive nursing care to clients in a community-based setting. The duties of this role vary by location but includes conducting health assessments, administering medications, implementing physicians' orders, and coordinating care with onsite and community healthcare providers. The Community Health Nurse monitors client health status, provides first aid and emergency response when needed, and educates clients and staff on medication management, disease prevention, and Narcan use. The position also involves developing and overseeing nursing care plans, ensuring a safe and hygienic environment, maintaining accurate clinical documentation and data reporting, and collaborating closely with program leadership and multidisciplinary staff to support client health, safety, and continuity of care.

What You Will Do:

\n

\n
• Collaborates with interdisciplinary teams to coordinate health and wellness activities.
\n
• Conducts client assessments to determine health needs, including taking vital signs as needed.
\n
• Accurately documents assessments, interventions, and outcomes in clients’ medical records.
\n
• Provides emergency care until EMS arrives and liaises with EMS personnel during medical emergencies.
\n
• Develops and implement individualized nursing care plans.
\n
• Health Promotion: Educates clients on disease prevention and health maintenance.
\n
• Assists in preparing statistical reports for funding sources (e.g., number of health assessments and referrals to primary care or specialists).
\n
• Care Coordination and Advocacy: Facilitates referrals and linkages to appropriate healthcare services based on client needs.

Licenses and Certifications: \n
\nLicense and current registration to practice as a Registered Professional Nurse in New York State requiredCPR/BLS Certification required \n

Education: \n
\nAssociate's Degree Nursing requiredBachelor's Degree Nursing preferred\n

Work Experience: \n
\nMinimum two years of experience working as an RN in LHCSA or CHHA requiredPrior experience working with behavioral health client population and providing clinical assessments of their health needs preferred \n

Pay Range \n

\nUSD $45.09 - USD $56.39 /Hr. \n
