Catholic Managed LTC Inc, part of ArchCare, is seeking a Community Health Nurse to join our Healthcare & Medical Services team. In this role, you will provide community-based nursing care, conduct health assessments, coordinate care plans, and educate patients and families on managing chronic conditions. You will collaborate closely with an interdisciplinary team to support patient-focused outcomes, ensure continuity of care, and connect individuals to needed services.
