Responsibilities Serve as the primary point of contact between the organization and healthcare providers across MI Driving referrals and admissions for a home care agency Building relationships throughout the healthcare space (hospital systems, SNFs, ALFs, discharge planners, case managers, social workers) Developing and maintaining referral pipelines Travel across Detroit counties to meet with key healthcare providers Conduct research to identify new providers in the region and explore opportunities for collaboration. #J-18808-Ljbffr Optify Search
