Minimum Requirements:

• Must have State License to Apply
• EPIC experience
• Medication Reconciliation for discharge medication list
• Communication skills for gathering prior to admission home medication list from patient, family, or pharmacy
• Communication skills for discharge counseling with patients/families
• Clinical knowledge of Antimicrobial Stewardship
• Interpretation of culture results for recommendations
• Knowledge of UTI, pneumonia, SSTI, etc. for recommendations
• Pharmacokinetic Dosing Management/Monitoring
• Anticoagulation Dosing Management/Monitoring
