Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours Experience: Minimum of 2 years acute care hospital case manager experience required. Experience in care coordination and discharge planning is necessary. License: CA state license required. Pending license accepted, but must be licensed upon start date. Certifications: N/A Must-Have: Candidates must have prior hospital experience. First-time travelers accepted. Weekend rotation is required. Must be flexible to meet departmental needs. Description: The position is for a registered nurse specialized in case management. The role involves care coordination and discharge planning. There are no floating expectations. Patient ratios are 1 nurse to 20 patients. Knowledge of drug and alcohol treatment options is essential. Experience with patients facing homelessness or mental health challenges is required. Candidates should have the ability to organize and participate in family meetings. The role includes developing treatment plans and arranging necessary services. Proficiency in computerized documentation and Microsoft applications is necessary. Candidates may be expected to float between nearby facilities if required. Refusal to float may impact the assignment. Providers may need communication devices to perform duties effectively. References are required covering experience within the last three years. Compliance with company policies and appropriate scheduling requests is essential. Completion of modules is required prior to start. All interactions with facility contacts must follow established protocols. Failure to adhere to scheduling commitments may lead to termination of the contract.
