Your Responsibilities As a Clinical Care Manager, you will: Oversee and coordinate care throughout the entire patient journey. Integrate patient care among clinicians, caregivers, and providers. Manage physician orders, documentation, and workflows within the electronic medical record (EMR) system. Collaborate with the Intake team to facilitate timely admissions and minimize referral delays. Conduct ongoing patient assessments and ensure alignment with the care plan. Review clinical reports (including occurrences, infection control, and on-call) and take necessary actions. Lead case conferences, recertifications, and discharge planning efforts. Ensure compliance with clinical, state, and federal regulations. Act as a key liaison among physicians, patients, families, and internal teams. Support care delivery through in-home visits as required. What We Are Looking For We are in search of RNs who appreciate the pace, accountability, and impact of home health care: Current RN license in the state of practice. Graduate from an accredited nursing program (BSN preferred). Experience in home health is highly desirable. Background in care coordination, case management, or clinical leadership is preferred. Strong clinical judgment with the ability to manage multiple priorities effectively. Confident communicator capable of collaborating across various disciplines.
