Home-Visit Community Health Advocate
Your Health Organization
Your Health Organization is seeking a dedicated Community Health Worker (CHW) to support patients in the Athens, GA area. The CHW will visit patients in homes and Assisted Living Facilities, facilitate visits with providers, and ensure timely access to medications and durable medical equipment. The role requires reliable transportation, a full-time schedule (M-F, 8:00 AM–5:00 PM), and strong communication skills to coordinate care and support families. #J-18808-Ljbffr
