Field Care Navigator & Community Health Advocate

UnitedHealth Group seeks a Field Care Coordinator to support LTSS members through care planning and coordination. This field-based role operates with a home office and includes substantial travel within MI to deliver person-centered, goal-oriented care. You will work with social workers and care teams to implement plans, conduct home visits, and connect members with community resources.
