Clinical Social Worker: Care Coordination & Family Support

Corewell Health seeks a part-time social worker to assist patients and families with complex psychosocial needs. You will collaborate with an interdisciplinary team to support discharge planning, referrals, and bereavement resources as needed. The role emphasizes crisis intervention, grief counseling, and management of identified abuse/neglect cases, with a required LMSW license in Michigan.
