Discharge & Care Coordination Specialist
Prime Healthcare
Garden City Hospital is seeking a case management professional to coordinate discharge planning and related activities. You will work under the supervision of a Case Manager/Social Worker, assisting with referrals, authorizations, and Medicaid submissions, to ensure safe, patient-centered planning. Required qualifications include knowledge of discharge planning and strong computer skills; preferred associates degree or LVN license, with prior hospital experience.
